• Livestock Injury Report (complete and submit 1 per show number even if there were no horse or cattle injuries)
• Event evaluation (make available to exhibitors, return completed forms with show results)
• Exhibitor representative (complete and submit with results if an exhibitor representative was named at the show)

  • Show managers will send one copy of the medication report form to AQHA with show results.
  • Exhibitor/owner should keep one copy of the form for their records.
